Ezidkhan

HealthWISE, funded by the Hunter New England Central Coast Primary Health Network (HNECCPHN), delivers the Ezidkhan Care Navigation Program to support Ezidi humanitarian entrants in Armidale. The program assists clients in identifying health goals and addressing barriers to health access and outcomes.

What Can We Assist With and Who Can Access Our Services?

The program provides:

  • Assistance in understanding and navigating the Australian healthcare system.
  • Appointment management, including reminders.
  • Referrals to appropriate health and social support services.
  • Assistance with IPTAAS forms and other health-related documentation.
  • Travel arrangements for out-of-town specialist appointments.

 

This service is available for Ezidi humanitarian entrants in the Armidale area.

How your patients can access these services

Access to this program is via GP referral only. Care Navigators work alongside GPs to provide non-clinical support, helping clients overcome barriers to healthcare. Follow-up support is available as required.
