Aftercare - Suicide Prevention Initiative

The Aftercare program is a free initiative aimed at providing person-centred psychological support to individuals and families affected by suicide. It focuses on reducing suicide risk and helping patients cope with the long-term emotional impact.

How Can this Service Help?

This program supports your patients by offering strength-based strategies and one-on-one psychological care, helping them manage their mental health following a suicidal event or the loss of a loved one. It complements ongoing mental health care and provides a continuity of support.

Available in Armidale, Glen Innes, Gunnedah, Inverell, Moree, Narrabri, Quirindi, Tamworth, Manilla, Barraba, and Walcha.

How your patients can access these services

For patients discharged after suicidal behaviour: A GP referral letter and a copy of their discharge papers are required. These can be faxed to 1300 452 059 or emailed to mentalhealth@healthwise.org.au. A Mental Health Treatment Plan is not necessary.

For family members or significant others: Self-referrals are accepted via phone at 1800 931 540.
