The Ezidkhan Care Navigation Program is a free service that helps members of the Ezidi community in Armidale with their health needs. Our Care Navigators support you in understanding and accessing healthcare services to improve your health and wellbeing.

What Can We Assist You With and Who Can Access Our Services?

Our Care Navigators can help you:

  • Understand how the Australian healthcare system works.
  • Manage your medical appointments and send you reminders.
  • Fill out forms for hospitals, Centrelink, and other health services.
  • Arrange travel for specialist appointments outside of town.
  • Connect you with other support services that can help.

 

This service is available for Ezidi community members living in Armidale.

How to Access Our Services

You will need a referral from your GP (doctor) to use this service. After the referral, our Care Navigator will meet with you to discuss your health goals and how we can support you. Follow-up meetings can be arranged if needed, either by phone or in person.
