Easy steps to make an appointment
- Make an appointment with your GP to get a referral to a HealthWISE mental health clinician.
- For some programs a GP referral is not required, and you can self refer.
- Your GP will write a Mental Health Treatment Plan which needs to be faxed (1300 452 059) or emailed
(mentalhealth@healthwise.org.au) to HealthWISE. The doctor’s surgery will be able to help with this. - HealthWISE will then contact you to organise your appointment.
- You may be eligible for free services. Please ask your GP.
When your GP has sent your referral to the Intake and Referral Service:
- Your referral will be triaged and sent to the most appropriate service
- If your referral is referred to HealthWISE:
- In rural and regional areas i.e. all New England North West areas EXCEPT Tamworth your service will be provided at no cost
- To access a free service in Tamworth you must either hold a concession card or deemed to be low income by your GP
You may also elect to access services privately (Fee for Service), in which case you will not need a Treatment plan from your GP. You will need to pay the full amount, medicare rebates may apply.
Payment method:
- Payment will be via EFTPOS (all credit and debit cards accepted). No cash or cheque payments will be accepted.
