Referring to HealthWISE Mental Health Services

To access HealthWISE Mental Health Services in NSW please direct your referral to the Head to Health Statewide Central Intake and Assessment Hub Also known as Medicare Mental Health. The referral should state that you want the client allocated to HealthWISE.
Referrals can be submitted via the following options;.
Head to Health Digital Online Referral Portal – (https://app.consultmed.co/refer/hpc)
Medical Objects – (ID – TH229200000T)
Integrating ConsultMed into your medical software. This allows patient information to be pre-populated in the form.
Call Head to Health Hub – 1800 595 212 (Monday to Friday 8.30am to 5pm)
For support to integrate ConsultMed into your medical software, please complete the contact form on their website (www.consultmed.co/contact/).

It is important to note that PHN commissioned mental health services will continue to be delivered by the existing local providers.

GP referrals are not needed for many HealthWISE mental health services.
Our mental health services are FREE.
Face to face and telehealth services delivery methods available.
HealthWISE has a team of highly qualified and experienced mental health clinicians that can treat and support individuals with a range of mental health conditions.
Services are available in the New England Northwest region.
HealthWISE is not a crisis service. If you or someone you know is at immediate risk, call 000 or attend your local emergency department.
To access HealthWISE Mental Health Services clients will be required to call the Head to Health Statewide Intake and Assessment Hub also known as Medicare Mental Health on 1800 595 212.
Alternatively clients can be referred by their GP to the Head to Health Statewide Intake and Assessment Hub.
Complete the intake assessment and ask for the referral to be directed to HealthWISE.
The Hub is available Monday to Friday from 8.30am to 5pm.
PHN commissioned mental health services will continue to be delivered by the existing local providers.
There are NO anticipated changes to service delivery for HealthWISE contractors.
