If you live in the Moree Local Government Area, we can help you manage your diabetes. Our Diabetes Shared Care program is designed to suit your needs and help to coordinate your care.  

The HealthWISE care manager will work with other health professionals to help you follow your GP Management Plan and Diabetes Care Plan.  

What does diabetes affect? It affects feet, teeth & gums, immune system, nervous system, kidneys, cardiovascular system, skin, and eyes.

Who Can Access Services?

A person living in the Moree Local Government Area (LGA) with diabetes, including: Type 1, Type 2 and Latent Onset Diabetes in Adults (LADA).

Talk to your GP about your eligibility and for a referral to the Diabetes Shared Care program. This program is supported through funding from the Hunter New England and Central Coast Primary Health Network.
